In April 2019 the patient A.F., a 72-year-old man, appears in the studio, complaining of annoyances and instability to the total removable upper prosthesis that he has been wearing for several years.
He also asks about the possibility of being able to wear a fixed prosthesis.
In the anamnesis, the patient appears to be being treated with an anticoagulant drug as a form of prevention for cardiovascular diseases.
The clinical analysis shows thick gingiva and a satisfactory height of keratinized gingiva.
The three-dimensional radiography analysis (CBCT) reveals a minimum bone height of 15 mm in the entire arch, and a minimum thickness of 7 mm.
An implant rehabilitation with immediate loading is suggested, with the insertion of 6 Prama implants. The bone dimensions allow flapless surgery to be performed without a surgical template and therefore immediate loading with definitive prostheses, without having to fear any bone and therefore gingival resorption consequent to traditional surgery.
This intervention made it possible to minimize the patient's discomfort, also limiting the bleeding that could have occurred with a classic surgery considering the patient's drug therapy with anticoagulant.
An orthopantomography 6 months after loading shows the success of the therapy and the maintenance of the marginal bone around implants.
