A 51 year old patient with no previous medical history arrives at the dental office with element 1.1 fractured horizontally. During the analysis the fracture of the 1.1 element root, the absence of element 2.2 and zirconium crowns on elements 1.2 and 2.1 are observed; there are no signs or symptoms of infection. Being a delicate aesthetically area, where the prosthetic emergence of the implant will determine the aesthetic result of the final reconstruction and where, additionally, the proximity of the adjacent teeth roots makes it difficult the positioning of the implant, it is decided to insert the implant using the more recent techniques of guided surgery. The non traumatic extraction of what is left of the root of 1.1 and the immediate positioning of the implant is planned according to the consensus published by Morton et al. in 2014. The planning is realized by means of the 3Diagnosys computer system (3DIEMME, Cant., CO, Italy) using the guided surgery kit EchoPlan (Sweden & Martina, Padua, Italy) for the next PRAMA implant insertion (Sweden & Martina, Padua, Italy).
