The aim of this retrospective study was the radiographic analysis of 137 implants, including 106 Pilot implants placed both in the jaw and the mandible using the E.R.E. (Edentulous Ridge Expansion) technique. The three-year radiographic analysis revealed a crestal bone gain of +0.89 mm (SD 0.39 mm), which the authors largely attributed to 3 clinical parameters: thickness of keratinised gingiva, use of dental implants with a 2-mm machined neck and insertion axis of the implants perpendicular to the occlusal surface of the antagonist. After 7 years, the follow-up X-ray revealed further bone gain: + 0.99 mm (SD: 0.50 mm).

138 implants were inserted in 50 patients in the maxilla with bone type D3 or D4. Osteotomes with a hand mallet were used in the Control Group, the Magnetic Mallet in the Test Group. At 24 months the implant survival in the Control Group was 92.75%, in the Test Group 97.10%. The bone gain obtained in all patients remained stable at 24 months in both groups. The patients who underwent operation with the Magnetic Mallet did not report any complications linked to benign paroxysmal positional vertigo, which instead was encountered in two patients operated with osteotomes with a hand mallet.

PROSPECTIVE STUDY ON THE MAINTAINING OF THE VERTICAL BONE VOLUME IN IMPLANTS INSERTED WITH THE M.I.S.E. TECHNIQUE

20 implants, inserted in 16 patients with simultaneous sinus lift performed with a transcrestal approach using the M.I.S.E. kit and with tricalcium sulphate as filling material, presented 100% survival at the 5-year follow-up. The increase of bone dimension was stable, the vertical bone reabsorption was reduced and comparable to that obtained using elevation techniques with opening of a side window.

The authors used magnesium-enriched hydroxyapatite (MHA) to fi ll the peri-implant defects in immediate sockets and observed that the bone recovery in the presence of this fi ller helped to better preserve the profi le of the bone crest in the absence of biomaterials. The difference was not significant.

This experimental study confirms the validity of the concept of distance osteogenesis, which has already been discussed in literature. Without the use of grafting materials, the bone can regenerate itself and occupy empty spaces, increasing in the direction of the implant surface and from the bottom upwards, in times proportional to the size of the defect.

This study identified the clinical finding that the presence of bone dehiscence, even severe, does not constitute a limitation or contraindication in implant-prosthetic therapy, provided that a suitable regeneration protocol is followed, whether using autologous bone or bovine bone.

The authors histologically verified in dogs the success of implants installed in the lower jaw in connection with the procedure of vertical and horizontal crest lift with autologous bone (control) and deproteinized bovine bone mineral (DBBM test). After 3 months, all the implants were stable. Those installed in autologous bone were completely integrated with the bone, while those in DBBM presented greater integration in the section in contact with parental bone.

This work studied the dimensional changes of tissues around the implants. After insertion of post-extraction implants in dogs with the use of deproteinized bovine bone mineral (DBBM test sites) to fill the coronal bone defects around the implant without the use of grafting material (control sites), impressions were taken at baseline and at 4 months. The impressions were casted and the models at the two different periods were compared with the use of three-dimensional measuring software. The histomorphometric aspects at 4 months were also compared. The test sites filled with DBBM had a much greater capacity to maintain the vertical buccal dimension compared to the control sites.

The authors histologically tested in dogs the effective behaviour of distance osteogenesis, artificially creating peri-implant gaps of different sizes around implants inserted 3 months after dental extraction. At one month from insertion, it was noted that the peri-implant defects were being filled with newly formed bone that grew from the bottom and from the walls of the defect towards the implant surface. The gap was more or less filled in relation to its initial size, no gap was completely reconstructed.
