This is a histologically analysis of bone healing in sites subjected to a sinus lift with autologous bone graft (test group) and with autologous/heterologous bone mixture (control group). No signifi cant differences were observed.

This article extensively illustrates a minimally invasive regenerative technique for inserting implants of Review an appropriate size and in a prosthetically favourable position into insuffi cient bone crest, preserving the surrounding tissues to obtain better aesthetic results. This procedure exploits the inherent capacity for second intention healing in order to avoid the use of graft material and membranes. Performed in one step surgery it allows achieving the desired three-dimensional remodelling of the hard and soft tissues around the implant: the opening of a partial thickness fl ap not only preserves vascularisation of the periodontal ligament favouring neo-osteogenesis, but also resolves natural root prominence, without affecting the papillae, the mucogingival junction and the position of the fenestration compared to the
adjacent elements. The 114 implants examined in this study suggest that the appropriate healing period for this technique is no more than 2 to 3 months, reducing discomfort for the patient.

The authors explain how this regenerative technique allows obtaining excellent aesthetic results with the repositioning not only of the bone crest but also of the entire bone-mucosa-gingival complex. This protocol allows inserting straight, non angled implants, therefore, with insertion axes that favour prosthetic rehabilitation.

In an article published in 2008, the authors apply the Edentulous Site Enhancement approach to implant treatment. The basic anatomical-physiological assumption is that the biological tissues, even though interfacing with an artifi cial element, maintain their physiological behaviour, especially with regard to the healing process, also including the defence mechanisms against pathogen attachments. In particular, an adequate vascular contribution to the tissues around the implant, like the one that is preserved with a partial thickness fl ap, guarantees adequate and stable bone and mucosal tissue trophism. Therefore, ESE sees the implant-prosthesis complex as a means to bring stable primary and secondary anatomic corrections to missing teeth and the surrounding tissues, underlying how the success of an implant is not limited to osseointegration, but also involves the predictability of the overall aesthetic result and its maintenance in the long term.

The article underlines the importance of a systematic and global approach to implant-supported prosthesis therapy. Appropriate planning, the choice of a surgical technique suitable for the regeneration and simultaneous insertion of the implants according to a favourable prosthetic axis allow reducing the surgical stages and optimising the functional and aesthetic results.

COMPARATIVE CLINICAL EVALUTATION ON THE USE OF A POLYLACTIC AND POLYGLYCOLIC ACID COPOLYMER IN THE SINUS LIFT

This study compared the rehabilitation protocols with an autologous/heterologous bone mix and PLGA in the sinus lift, with simultaneous implant insertion. Both procedures demonstrated good effi cacy and reliability.

The behaviour of the ZirTi surface is histologically evaluated in a series of sinus lift procedures with nanocrystalline hydroxyapatite grafting and simultaneous insertion of experimental micro-screws in titanium. After 3 months, the histologic control demonstrated well-mineralised bone and no connective tissue interposition.

This clinical study verifi ed the survival rate of implants in case of horizontal augmentation of the crest with the use of fresh-frozen bone and 7 different types of implants. The survival rate was 100% and the results showed that this regenerative technique allows the fi xtures to be loaded prosthetically even after 4 months with good results.
