The authors investigated the clinical and microbiological causes of peri-implantitis in 534 patients with 1507 implants (made by 10 different manufacturers), the 7.3% of whom showed signs of perimplantitis. In the patients that exhibited signs of peri-implantitis, the authors found:

• the presence of a high percentage of plaque;
• bleeding on probing;
• implants with less than 2 mm of attached gingiva were more prone to peri-implantitis.;
• a high percentage of the implants affected by peri-implantitis had been originally inserted with a regenerative therapy;
• there were no differences in microbiological load between peri-implant sulci and connections;
• there was a prevalence of implants with cement-retained crowns;
• the microbiological load around adjacent teeth had the same composition of as that around implants.

The aims of this review were 1) to analyse the clinical results of implants made with different forms, surfaces and materials, in different combinations, and 2) investigate whether implants with machined surfaces displayed more early failure compared to implants with a roughened surface. The analysis concluded that 1) there is no evidence that particular implants forms have greater success than others, and 2) there is limited evidence that implants with relatively smooth surfaces are less prone to bone loss due to chronic infections (peri-implantitis) than implants with a roughened surface 3 years after loading, although these appeared to be more frequently subject to early failure.

This three-year clinical study was carried out on 632 two-phase Premium implants in 252 patients. Cumulative survival was 98.73% and average peri-implant bone resorption was 0.8±0.03 mm, with a slightly greater trend in the maxilla with respect to the mandible, and significantly correlated to the site and patient age and gender (male or female).

In this study, the presence of keratinized gingiva with a thickness greater than 3 mm around 135 osseointegrated Pilot implants with a machined collar of 2 mm gave a vertical bone gain of 1.20 ± 0.49 mm in a follow-up of 9.71± 4.88 years.

In this retrospective analysis on 429 implants of 5 different types, Global implants showed greater survival and success, matched by less peri-implant bone loss compared to other implants.

This multi-centre four-year prospective study on 377 Global implants reported an implant survival rate of 99.7 % and a success rate of 99.47%. The DOCTA connection with a double internal octagon seems to favourably reduce the risk of prosthetic components becoming unscrewed.
The greater the diameter of the fixtures inserted, the greater primary stability will be. The same result is not necessarily obtained with longer implants.

IMPLANT POSITIONING AND RECONSTRUCTION OF HARD AND SOFT TISSUES: A SIMPLIFIED PROTOCOL

Infective endodontic or periodontal pathologies and root fractures can often cause serious bone defects, involving in particular the buccal wall.
Residual post-extraction bone volumes are therefore often affected, in both their apicocoronal and their vestibulopalatal dimensions, with evident deficits and asymmetries also involving the mucogingival tissues. The aim of this case report is to present a simplified protocol for the morphological restoration of hard and soft tissues simultaneously with the insertion of an implant in the aesthetic area.

41 patients received 70 Premium implants inserted with a flapless procedure versus opening of total-thickness flaps. Cumulative survival at 24 months was 98.57%, and with an average resorption of 1.19±0.9 mm.
Bone loss at 24 months was significantly higher in the group with total-thickness flaps. The preservation of marginal bone level with Premium implants was excellent.

PROSPECTIVE STUDY ON THE MAINTAINING OF THE VERTICAL BONE VOLUME IN IMPLANTS INSERTED WITH THE M.I.S.E. TECHNIQUE

20 implants, inserted in 16 patients with simultaneous sinus lift performed with a transcrestal approach using the M.I.S.E. kit and with tricalcium sulphate as filling material, presented 100% survival at the 5-year follow-up. The increase of bone dimension was stable, the vertical bone reabsorption was reduced and comparable to that obtained using elevation techniques with opening of a side window.

The use of the MISE (Minimal Invasive Sinus Elevation) technique, which calls for the use of high precision depth-calibrated rotating instruments, allowed the insertion of 64 implants with a success rate of 100% and significant recovery of the average vertical dimension at 18 months. Patients appreciated the minimal invasiveness of the protocol and did not feel pain or discomfort.
